Registrar's Office Forms

The following self-descriptive forms may be printed and completed, and either dropped off or faxed to the Registrar’s Office. Detailed instructions are included on each individual form.

Request a Transcript

Official transcripts are $3.00 each. If you are requesting your transcript by mail, you should enclose a check made payable to Yale School of Forestry & Environmental Studies. If you fax your request, please mail your payment as soon as possible after sending your fax. There is no charge for unofficial transcripts.

Your transcript will be generated and sent by regular mail within one week of our receiving your request. If you want your transcript sent by overnight delivery, there is an additional delivery charge of $10 for each U.S. address, and $25 for each international address to which a transcript is to be sent.

Degree Requirements
It is the responsibility of each student to ensure that all degree requirements have been met for graduation. Program Review Forms for each degree must be completed during the student’s final semester.
